Consent to the processing of personal data

CONSENT TO THE PROCESSING OF PERSONAL DATA

According to Article 7 of the EU General Data Protection Regulation 2016/679 (GDPR) and Act No. 110/2019 Coll., on the processing of personal data, as amended (the "Act")

➢ By checking this box, you agree to the processing of your personal data within the scope specified below ("Data") for the purposes of processing listed below:

A. Publication of personal data to the extent of name and surname as a donor to a public collection, along with the donated amount on promotional channels and in the foundation's annual report, as well as on the recipient’s promotional channels – Your personal data may be used for the purpose of disclosing the fact that you have contributed to this public collection and the concrete amount of your donation in promotional materials, documents, or other promotional channels, and in the foundation’s annual report, as well as in the promotional materials, documents, or other promotional channels of the donation recipient;

B. Provision of personal data to the extent of name, surname, donated amount, and contact email address to the recipient of the donation – Your personal data may be provided to the recipient of the donation for the purpose of informing them about who has made a donation and in what amount, including providing your email address in case the recipient wishes to contact you (e.g., to express gratitude);

C. Sending information about the foundation’s activities and initiatives (marketing purposes) – Your personal data, including name, surname, email address, contact address, and phone number, may be used to send you updates about the foundation’s activities and initiatives, including ongoing and planned programs and projects, related events and occasions, as well as invitations to these events, including via electronic communication (newsletters containing relevant updates);

D. Inclusion of your personal data (name, surname, date of birth, contact address, email address, contact phone number) in the internal contact database – Your personal data will be stored in an internal contact database (CRM) maintained for the purpose of providing information about the foundation’s activities and initiatives, sending birthday and holiday greetings, and recognizing significant days in the Czech Republic. To enhance collaboration across partner organizations (Akademie klasické hudby, z. ú., and KKCG Services a.s.), these contacts may be shared within the database.

This consent to data processing is granted to Karel Komárek Family Foundation, which is the data controller under the Act:

Nadace Karel Komárek Family Foundation, a foundation with its registered seat at Evropská 866/71, Vokovice, 160 00 Prague 6, ID No.: 06212093, registered in the Foundation Register maintained by the Municipal Court in Prague under File No. N 1525.

Karel Komárek Family Foundation, in line with its public benefit purpose, supports projects that have the potential to inspire individuals and communities to find the courage to bring their plans and visions to life, particularly in the areas of cultural project financing, environmental protection, education, science and research, new technologies, and care for the sick and disabled, as well as in any field where courage commands our respect.

Your consent is granted to the above‑mentioned controller(s) until revoked.

Your data, provided based on this consent, is processed both automatically and manually and is stored mainly in electronic form (only exceptionally also in paper form). However, the foundation does not engage in automated decision‑making, including profiling, within the processing of your personal data. We take all necessary technical and organizational measures to ensure its protection.

RECEIVING INFORMATION ABOUT PARTNER ORGANIZATIONS’ ACTIVITIES – INFORMATION ON ACTIVITIES, EVENT INVITATIONS TO EVENTS AND ACTIVITIES ORGANISED BY THEM AND BENEFIT FROM THE ADVANTAGES OF THESE PARTNER ORGANISATIONS:

➢ In addition to granting consent to Karel Komárek Family Foundation, you also consent to the processing of your personal data in the above‑specified scope, for the above‑mentioned purposes, and for the above‑mentioned duration, i.e. to the extent and under the terms of the consent granted to Karel Komárek Family Foundation,  by the following entities, which will also be considered data controllers under the Act:

Akademie klasické hudby, z.ú., with its registered seat at Maiselova 25/4, Staré Město, 110 00 Prague 1, ID No.: 26725347, registered in the Register of Institutes maintained by the Municipal Court in Prague under File No. U 527;

KKCG Services a.s., with its registered seat at Evropská 866/71, Vokovice, 160 00 Prague 6, ID No.: 27107744, registered in the Commercial Register maintained by the Municipal Court in Prague under file number B 9040.
